Position Summary:
The Community Habilitation Coordinator is responsible for overseeing, coordinating, and ensuring the delivery of high-quality community habilitation services to individuals enrolled in Home and Community-Based Services (HCBS) waiver programs. This role involves developing person-centered plans, supervising direct support staff, monitoring service outcomes, and ensuring compliance with all state, federal, and agency regulations.
KEY ESSENTIAL FUNCTIONS: DUTIES AND RESPONSIBILITIES:
Service Coordination & Oversight

Develop, implement, and maintain person-centered Community Habilitation service plans based on individual goals, needs, and preferences.

Coordinate initial and ongoing assessments to identify strengths, needs, and skill-building opportunities.

Ensure that services are delivered in accordance with the individual’s Life Plan, habilitation plan, and regulatory requirements.

Monitor service delivery through home visits, community observations, staff consultations, and documentation review.

Perform other related duties as assigned by the Program Director.

Staff Support & Supervision

Train, support, and provide guidance to Direct Support Professionals (DSPs) assigned to Community Habilitation cases.

Ensure staff understand habilitation goals, teaching strategies, safety protocols, and documentation expectations.

Address performance concerns and provide constructive feedback to promote high-quality service delivery.

Perform other related duties as assigned by the Program Director.

Case Management & Documentation

Maintain accurate, timely, and comprehensive documentation, including progress notes, service logs, data collection, workflow trackers and monthly summaries.

Track individual progress toward community habilitation goals and adjust plans as needed.

Ensure compliance with all HCBS waiver guidelines, agency policies, and audit standards.

Perform other related duties as assigned by the Program Director.

Collaboration & Communication

Serve as the primary point of contact for individuals, families/guardians, support teams, and external providers.

Attend care team meetings, Life Plan meetings, and interdisciplinary conferences as needed.

Communicate changes in service needs, staffing updates, and concerns to the Program Director and team members promptly.

Perform other related duties as assigned by the Program Director.

Quality Assurance & Compliance

Ensure all services adhere to OPWDD (or applicable state agency) regulations and HCBS waiver requirements.

Participate in internal audits, quality improvement initiatives, and trainings mandated by the agency.

Ensure proper incident reporting and follow-up in accordance with agency and state guidelines.

Perform other related duties as assigned by the Program Director.

Minimum Qualifications:
-
Bachelor’s degree in Human Services, Psychology, Education, Social Work, or a related field (preferred).

Experience working with individuals with intellectual and developmental disabilities (IDD).
-
Knowledge of HCBS waiver services and person-centered planning.
-
Strong organizational, communication, and problem-solving skills.
-
Ability to work independently, manage multiple cases, and adapt to individual needs.

Valid driver’s license and reliable transportation (not a requirement but preferred).

Ability to pass background checks and agency-required trainings (e.g., SCIP, CPR/First Aid).
