POSITION SUMMARY
Unique Care Community Services (UCCS) is seeking a qualified and experienced Speech-Language Pathologist (SLP) to provide comprehensive speech and
language services to individuals with developmental delays and disabilities.
The SLP will conduct evaluations, develop individualized treatment plans, and provide direct therapy services while collaborating with families and
interdisciplinary teams. All services must be delivered in compliance with Early Intervention Program (EIP) and OPWDD HCBS Waiver standards, as well
as Medicaid requirements.

PRIMARY RESPONSIBILITIES

1. Evaluation & Assessment

Conduct comprehensive speech and language evaluations, including:
   -Receptive and expressive language
   -Articulation and phonological development
   -Oral-motor functioning
   -Feeding and swallowing assessments (if applicable)

-Participate in Multidisciplinary Evaluations (MDEs) when required
-Identify communication deficits and functional needs
-Prepare detailed evaluation reports with clinical recommendations

2. Treatment Planning

-Develop individualized treatment plans aligned with:
  -IFSP (Early Intervention) outcomes
  -ISP/Life Plan (OPWDD) goals

-Establish measurable goals targeting:
   -Communication development
   -Social/pragmatic language skills
   -AAC (Augmentative and Alternative Communication) use

-Update plans based on participant progress and reassessment

3. Service Delivery

-Provide direct speech therapy services in:
   -Home settings
   -Daycare/early childhood programs
   -Community environments

-Utilize evidence-based and developmentally appropriate interventions
-Incorporate play-based and naturalistic strategies for EI participants
-Provide caregiver coaching to support skill generalization
-Collaborate with ABA therapists, OT, PT, and other providers

4. Documentation & Reporting

-Complete all required documentation, including:
   -Session notes (within 24–48 hours)
   -Evaluation reports
   -Progress reports
   -Treatment plans and updates

-Ensure documentation meets:
    -Medicaid billing standards
    -Early Intervention and OPWDD requirements
    -Maintain audit-ready records at all times

5. Collaboration & Team Participation

-Participate in:
   -IFSP meetings (Early Intervention)
   -ISP/Life Plan meetings (OPWDD)

-Maintain ongoing communication with:
   -Families and caregivers
   -Service coordinators and care managers
   -Interdisciplinary team members

6. Compliance & Ethics

-Adhere to all applicable regulations and standards, including:
    -New York State Department of Health Early Intervention requirements
    -Office for People With Developmental Disabilities HCBS Waiver standards
    -Medicaid billing and documentation regulations
    -HIPAA confidentiality requirements

-Maintain ethical and professional conduct at all times

MEDICAID BILLING RESPONSIBILITIES

-The SLP is responsible for ensuring that all services are:
     -Medically necessary
     -Authorized prior to delivery
     -Accurately documented

-Services may include:
     -Evaluations
     -Individual therapy sessions
     -Group therapy sessions (if applicable)

-All billing must comply with Medicaid standards and be supported by complete documentation.

MINIMUM QUALIFICATIONS
Required

-Master’s Degree in Speech-Language Pathology
-Active NYS License as a Speech-Language Pathologist
-Minimum 2+ years of experience working with:
   -Early Intervention (Birth–3) preferred
   -Individuals with developmental disabilities

Preferred
-Certificate of Clinical Competence (CCC-SLP)
-Experience with:
    -Autism Spectrum Disorder (ASD)
    -AAC systems (PECS, communication devices)
    -Feeding and swallowing disorders

-Bilingual (Spanish or other languages)

REQUIRED CREDENTIALS & CLEARANCES

-Active NYS SLP License (in good standing)
-Professional Liability Insurance ($1M/$3M minimum)
-CPR/First Aid Certification
-Mandated Reporter Training (NYS)
-Background clearance

COMPENSATION

-Competitive hourly rate: $75 – $120 per hour (based on experience)
-Compensation structure:
   -Per session / per billable hour

-Payment contingent upon:
    -Completed documentation
    -Verified service delivery
    -Medicaid compliance

WORK SCHEDULE
-Flexible scheduling based on assigned cases
-Evening and weekend availability preferred

WORK ENVIRONMENT

-Services provided in:
   -Homes
   -Daycare centers
   -Community settings
-Travel required within service areas

PHYSICAL REQUIREMENTS

   -Sit, stand, and move during sessions
   -Engage with children and adults in therapeutic activities
