Job Title: HCBS/Children’s Waiver Coordinator Department: All
Reports to: Program Director FLSA Status: Non-Exempt
Classification: Full-time Date: 9/4/2024
Pay Rate: $65k/yr
Position Summary:
We are seeking a detail-oriented and compassionate individual to fulfill the role of
HCBS/Children’s Waiver Case Coordinator. The Case Coordinator will be responsible for coordinating
services for clients, maintaining case records, and facilitating communication between clients, families,
and service providers. The ideal candidate should possess strong organizational skills, excellent
communication abilities, a strong knowledge of HCBS/Children Waiver Services, and a passion for
helping others.
KEY ESSENTIAL FUNCTIONS: DUTIES AND RESPONSIBILITIES:
· Coordinate and facilitate services for clients.
· Maintain accurate and updated case records, ensuring completeness and confidentiality.

· Communicate effectively with clients, families, and service providers to ensure that the
needs of clients are met.
· Assist in developing and implementing case management plans in collaboration with
program director, clients and other stakeholders.
· Conduct fieldwork to support families.

· Provide support and guidance to clients and their families throughout the case management
process
· Collaborate with other professionals, such as social workers and healthcare providers, to
ensure comprehensive care for clients.
· Attend all training and meetings offered by the Department of Health (DOH) regarding
HCBS/Children’s Waiver.
· Attend meetings with team on an annual, semi, and when needed
· Collaborate with Director of Program Development to become approved to provide more
service under HCBS/Children’s Waiver.
· Responsible for intake, hiring, training and supervision of Waiver Service providers

Minimum Qualifications:
·Bachelor's degree in social work, psychology, or a related field.
·Driver’s License (NY State in Good Standing)
· Proven experience in HCBS/Children’s Waiver (a must), case management, social services, or a related field may be preferred.
·Strong organizational and time management skills.
· Excellent written and verbal communication skills.
· Empathy and compassion for the clients and families being served.
· Ability to work effectively both independently and as part of a team.
· Proficiency in relevant computer applications and software.
