Position Summary:

 

Unique Care Community Services (UCCS) is seeking a licensed and experienced Occupational Therapist (OT) to provide high-quality, evidence-based occupational therapy services to individuals with developmental delays and disabilities.
The OT will conduct evaluations, develop individualized treatment plans, and provide direct therapy services to improve functional independence, sensory processing, and daily living skills.

 

Services must be delivered in compliance with Early Intervention Program (EIP) and OPWDD HCBS Waiver standards, as well as Medicaid regulations.

Primary Responsibilities:

 

1. Evaluation & Assessment

   

 Conduct comprehensive occupational therapy evaluations, including:

• ​Sensory processing and integration

• Activities of Daily Living (ADLs)

• Visual-motor integration

• Self-regulation and adaptive functioning

• Participate in Multidisciplinary Evaluations (MDEs) when required

• Identify functional limitations and developmental delays

• Prepare detailed evaluation reports with clinical recommendations

 

2. Treatment Plan

Develop individualized treatment plans aligned with:

• IFSP (Early Intervention) outcomes

• ISP/Life Plan (OPWDD) goals

 

Establish measurable goals addressing:

• Fine motor development

• Sensory integration

• Self-care and daily living skills

• Environmental adaptations

 

Update plans based on progress and reassessment

 

3. Service Delivery

• Provide direct occupational therapy services in:

            -Home settings
            -Daycare/early childhood programs
            -Community environments

• Utilize evidence-based and developmentally appropriate interventions

• Implement play-based, sensory-based, and functional activities

• Provide caregiver coaching to support skill generalization

• Recommend adaptive equipment and environmental modifications

• Collaborate with ABA, Speech, PT, and other providers

 

4. Documentation & Reporting

• Complete all required documentation, including:

       -Session notes (within 24–48 hours)
       -Evaluation reports
       -Progress reports
       -Treatment plans and updates

 

• Ensure documentation meets:

       -Medicaid billing requirements
       -Early Intervention and OPWDD standards

 

• Maintain accurate, complete, and audit-ready records

  

5. Collaboration & Team Participation

• Participate in:

       -IFSP meetings (Early Intervention)
       -ISP/Life Plan meetings (OPWDD)

• Maintain consistent communication with:

        -Families and caregivers
        -Service coordinators and care managers
        -Interdisciplinary team members

6. Compliance & Ethics

•  Adhere to all applicable regulations and standards, including:

        -New York State Department of Health Early Intervention requirements
        -Office for People With Developmental Disabilities HCBS Waiver standards
        -Medicaid billing and documentation regulations
        -HIPAA confidentiality requirements
 

• Maintain ethical and professional conduct at all times

MEDICAID BILLING RESPONSIBILITIES

The OT is responsible for ensuring all services are:
      -Medically necessary
      -Authorized prior to delivery
      -Properly documented

Services may include:

• Evaluations

• Individual therapy sessions

• Group therapy sessions (if applicable)

 

All billing must comply with Medicaid standards and be supported by complete documentation.

MINIMUM QUALIFICATIONS
Required

 

• Master’s or Doctorate Degree in Occupational Therapy

• Active NYS License as an Occupational Therapist

• Minimum 2+ years of experience working with:

        -Early Intervention (Birth–3) preferred
        -Individuals with developmental disabilities

Preferred

 

• Experience with:

       -Autism Spectrum Disorder (ASD)
       -Sensory integration techniques
       -Adaptive equipment and environmental modifications

 

• Bilingual (Spanish or other languages)

 

REQUIRED CREDENTIALS & CLEARANCES

 

• Active NYS OT License (in good standing)

• Professional Liability Insurance ($1M/$3M minimum)

• CPR/First Aid Certification

• Mandated Reporter Training (NYS)

• Background clearance

 

COMPENSATION

• Competitive hourly rate: $70 – $110 per hour (based on experience)

• Compensation structure 

       -Per session / per billable hour

 

• Payment contingent upon:

       -Completed documentation
       -Verified service delivery
      -Medicaid compliance

WORK SCHEDULE

• Flexible scheduling based on assigned cases

• Evening and weekend availability preferred

 

WORK ENVIRONMENT

• Services provided in:

       -Homes
       -Daycare centers
       -Community settings

• Travel required within service areas

PHYSICAL REQUIREMENTS

• Ability to:

       -Sit, stand, and move throughout sessions
       -Assist participants with physical activities when needed
       -Demonstrate therapeutic exercises and interventions
