Contract Job Description
Position Type: Independent Contractor (1099) Physical Therapist
Reports to: Clinical Director/ CEO
Location: Community-Based/ Agency Offices

Service Area: Early Intervention,
(Birth- 3), OPWDD (Children & Adults)
PRIMARY RESPONSIBILITIES
1. Evaluation & Assessment
-
Conduct comprehensive physical therapy evaluations, including:
-Gross motor development
-Muscle strength and tone
-Balance, coordination, and posture
-Range of motion and mobility
-Functional movement and gait analysis
-
Participate in Multidisciplinary Evaluations (MDEs) when required
-
Identify physical and developmental delays impacting mobility and independence
-
Prepare detailed evaluation reports with clinical recommendations
2. Treatment Planning
-
Develop individualized treatment plans aligned with:
-IFSP (Early Intervention) outcomes
-ISP/Life Plan (OPWDD) goals
-
Establish measurable goals addressing:
-Gross motor skill development
-Mobility and functional independence
-Strength, endurance, and coordination
-Postural control and alignment
-
Update plans based on participant progress and reassessment
3. Service Delivery
-
Provide direct physical therapy services in:
Home settings
Daycare/early childhood programs
Community environments
-
Utilize evidence-based and developmentally appropriate interventions
-
Implement therapeutic exercises, mobility training, and positioning strategies
-
Provide caregiver training to support carryover of skills
-
Recommend assistive devices, orthotics, and adaptive equipment when needed
-
Collaborate with ABA, OT, Speech, and other providers
4. Documentation & Reporting
-
Complete all required documentation, including:
-Session notes (within 24–48 hours)
-Evaluation reports
-Progress reports
-Treatment plans and updates
-
Ensure documentation meets:
-Medicaid billing requirements
-Early Intervention and OPWDD standards
-
Maintain accurate, complete, and audit-ready records
5. Collaboration & Team Participation
-
Participate in:
-IFSP meetings (Early Intervention)
-ISP/Life Plan meetings (OPWDD)
-
Maintain ongoing communication with:
-Families and caregivers
-Service coordinators and care managers
-Interdisciplinary team members
6. Compliance & Ethics
-
Adhere to all applicable regulations and standards, including:
-New York State Department of Health Early Intervention requirements
-Office for People With Developmental Disabilities HCBS Waiver standards
-Medicaid billing and documentation regulations
-HIPAA confidentiality requirements
-
Maintain ethical and professional conduct at all times
MEDICAID BILLING RESPONSIBILITIES
The PT is responsible for ensuring that all services are:
-
Medically necessary
-
Authorized prior to delivery
-
Properly documented
Services may include:
-
Evaluations
-
Individual therapy sessions
-
Group therapy sessions (if applicable)
All billing must comply with Medicaid standards and be supported by complete documentation.
MINIMUM QUALIFICATIONS
Required
-
Doctorate or Master’s Degree in Physical Therapy
-
Active NYS License as a Physical Therapist
-
Minimum 2+ years of experience working with -Early Intervention (Birth–3) preferred
-Individuals with developmental or physical disabilities
Preferred
-
Experience with:
-Pediatric physical therapy
-Neuromuscular and developmental disorders
-Assistive technology and mobility devices
-
Bilingual (Spanish or other languages)
REQUIRED CREDENTIALS & CLEARANCES
-
Active NYS PT License (in good standing)
-
Professional Liability Insurance ($1M/$3M minimum)
-
CPR/First Aid Certification
-
Mandated Reporter Training (NYS)
-
Background clearance
COMPENSATION
-
Competitive hourly rate: $75 – $115 per hour (based on experience)
-
Compensation structure:
-Per session / per billable hour
-
Payment contingent upon:
-Completed documentation
-Verified service delivery
-Medicaid compliance
WORK SCHEDULE
-Flexible scheduling based on assigned cases
-Evening and weekend availability preferred
WORK ENVIRONMENT

Travel required within service areas
PHYSICAL REQUIREMENTS

Ability to:
-Lift, assist, and position participants safely
-Demonstrate exercises and mobility techniques
-Stand, walk, and move for extended periods
