Day-Habilitation Supervisor 

JOB DESCRIPTION

Reports to: Director Of Program         FLSA Status: Non-Exempt

Classification: Full-Time                       Pay Rate: $25/hour

 

Position Summary:
 
The Habilitation Coordinator (Supervisor) is responsible for the development of Habilitation Plans and Individualized Plans for Protective Oversight.

The supervisor works in collaboration with room supervisor and the day-habilitation staff on implementation of individualized plans. They are also responsible for the completion of Monthly Summary reports and functional assessments for individuals utilizing the service.

In addition, the Day Habilitation Supervisor assists individuals receiving services in acquiring and/or maintaining the skills, personal habits and positive attitudes that will enable them to maintain and/or enhance independence. Accurate program and statistical records are expected to be maintained, including but not limited to: daily tracking and monthly progress notes, as well as any other required documentation in accordance with agency and regulatory requirements.

Supervisors will provide general supervision of individuals receiving services in the cafeteria and hallways during lunch and breaks, and they will provide oversight of individuals dietary needs. 


Essential Functions and Responsibilities:
 

  • Develops and provides structured services and skill building activities designed to meet the needs of individuals receiving services in accordance with their valued outcomes and assist in the scheduling and planning of same.

  • Participates in the development and directly implements the day habilitation plan for individuals receiving services to reflect their interests, capabilities, and needs, and to enhance their skills and level of independence.

  • Encourages individuals to express and develop their individual interests.

  • Provides individuals with opportunities to maintain/ enhance their existing skills, overall functioning and independence, and sense of belonging and community integration through various in-house and community activities.

  • Assists individuals in achieving their valued outcomes by providing prompts, reminders, guidance, and role modeling in areas such as socialization, personal hygiene, exercise, nutrition, behaviors, etc., while involved in skill building activities.

  • Provide appropriate physical care of assigned individuals to maintain their cleanliness, safety, and comfort. This would include tasks such as direct assistance with washing, dressing and toileting, and range of motion. This also includes monitoring, correcting, and modeling appropriate behaviors at lunch and break times.

  • Ensures safety and welfare of individuals in their movement and transport.

  • Develops individual specific programs and provides immediate feedback on performance.

  • Transports individuals receiving services using Agency vehicles.

  • Takes initiative and works independently or with other persons within program, without supervisory directions.

  • Attends ISP meetings as needed.

  • Provides self-esteem building opportunities for persons with memory loss through creative art projects such as painting, drawing, and designing.

  • Reviews and updates Habilitation Plans, to meet regulatory body guidelines at least twice yearly or as needed or requested by the Individual.

  • Coordinates the writing and implementation of Individual Plans of Protection.

  • Trains program staff regarding Individual’s plans.

  • Evaluate the effectiveness of plans through monthly summary reports and functional assessments.

  • Provides support for Individuals in skill development.

  • Promotes Individual’s feelings of self-respect and respect for others, as well as a sense of connection to the community.

  • Develops meaningful partnerships with the Individuals, families, and the community to fulfill the individual’s right to live productive and rewarding lives and to ensure all doors are open and opportunities are available.

  • Completes and maintains all records in accordance with the standards of regulatory agencies governing the operation of Unique Care Community Services

  • Meet with Supervisory team on a regular basis for exchange of information, brainstorming, and problem solving.

  • Participates in the team decision-making process and carries out decisions consistently.

  • Presents a positive and professional public image. Is polite and professional with all people in daily interactions.

  • Adheres to Agency & Program policy and procedures and serves as a role model for other program staff.

  • Achieves program and department goals by performing other responsibilities as required.

  • Maintains confidentiality of all protected health information in accordance with agency policy

  • Participates in quality and fiscal program audits, implementing recommendations, as needed.

Qualifications:
 

Bachelor’s degree in education, Social Work, Human Services or related field required. Two years’ experience in working with people with developmental disabilities preferred.

  • Valid NYS driver’s license and clean driving record required.

  • Must have demonstrated organizational skills.

  • Must have demonstrated interpersonal skills.

  • Must have excellent oral and written communication skills.

  • Must have computer literacy skills and a demonstrated ability to adapt to new computer software.

  • Possess good time management skills.

  • Ability and desire to work as part of an interdisciplinary team.

  • Moderate level of written and oral communication skills

  • High level of interpersonal skills

  • Knowledge of behavioral techniques and developmental disabilities to assist in the care of
